Many possibilities : Could be chest could be muscle or spine. Outside chance atypical presentation gallbladder issue. Suggest doctor examination and assessment as may need tests run, should be urgently if breathing difficulties

Pain : Pain on left side of rib radiating to back could be due to pancreatitis , reflux , etc . Need further assessment , examination and tests . Please consult a doctor.
