I suspect : a lung problem that involves the chest lining or pleura near the diaphragm (muscle that aids inspiration/expiration). Diaphragm inflammation pain is often "referred" to the upper chest or shoulder. You might have pneumonia, bronchitis resulting in pleurisy or even a pulmonary embolus/blood clot.
