Endodontist: As your Dentist has been unsuccessful in treating your pain, probably time to seek treatment by a specialist Endodontist. Your Dentist should be able to refer one in your area. As to not being helped in ER, that's not unusual. Just as going to Dentist for acute kidney infection would be non-productive, going to Physician for tooth infection would be non-productive.

Second opinion: Go for a second opinion to rule out all dental etiology. A hospital emergency room, unless there is a dental residency program, offers only palliative treatment, generally. There may be a chance that the pain is sinus related, if this is an upper tooth. Start with the simplest first.
