Urologist: These symptoms can be caused by obstructive kidney problems. Is a urologist involved in this case? If so, are further studies scheduled? In my opinion the working diagnosis here is obstruction somewhere in the involved kidney or drainage system and the workup and treatment should proceed expeditiously.
