Possibly: But can't be determined for sure without an examination. There are dozens of dental/medical/combined reasons for bad breath. As most often problem is dental, see your Dentist for an examination.

Not necessarily : While bad breath is often caused by tooth decay and gum disease, other reasons can include food allergies, other stomach issues, tonsil stones, post nasal drip and sinus infections. To cure start with good oral hygiene and then a dental visit to treat any tooth and gum issues. If that doesn't solve the problem then have tonsils and sinuses evaluated.
