Splenomegaly: Yes, splenomegaly can cause referred pain to the left shoulder due to irritation of the adjacent diaphragm and the anatomic course of the associated phrenic nerve. If the pain is acute, it can be a sign of splenic rupture(Kehr's sign), which can be spontaneous in the setting of slpenomegaly. If this is the case, seek medical attention emergently.
