Get evaluated.: From what you described, it is urgent that you go see your doctor for an evaluation. You likely don't have liver disease, however you might have some other issue, like a blood disorder like leukemia/lymphoma that could be causing your symptoms of fatigue and your enlarged spleen and liver. Your shoulder pain might be a separate issue, but it also could be part of this. Again please see your dr.

Serious: You should see your doctor asap. You are describing something serious, liver is mafunctioning secondary to something else. Pain in the shoulder is radiating there from something in the abdomen. Any weight loss, night sweats? You need an abdominal ct and blood tests.
