Learn to relax: It seems you have been evaluated by doctors and they found nothing wrong with your heart, lung or ent.You have to learn to relax and realize that the doctors did not find anything wrong.Follo up with your family doctor with your concen,It seems it may be anxiety problem more than physical disease.Discuss with your doctor your concern and learn to relax

See a cardiologist : If you are concerned that your symptoms are related to your heart then you should see a Cardiologist who can arrange an Echocardiogram for you.

Anxiety : Anxiety disorder is extremely common. What you describe sounds like this rather than cardiopulmonary disease. If your doctor agrees, psychology help may avail you.

Please clarify but:: Not sure what you mean by "doc ruled out?" If you haven't seen those docs then do. If you have, you may have physical issues not yet discovered. This can be due to magnesium deficiency. Try mag. taurate 250-400 mg/day, CoQ10 100 mg, consider Hawthorne and Resveratrol. Consider seeing integrative doc and acupuncture. Infections like Babesiosis can cause this too. Agree that anxiety may be a factor.
