A 24-year-old male asked:
tonsils used to be fine. they've now grown to be severely enlarged (grade 4) meeting at midline. negative for mono. what else could cause this?

Dr. Donald Colantino answered

Otolayngologist: Since both tonsils are hypertrophied and if you have no evident enlarged lymph glands or abnormality of your cbc, I would suspect some systemic cause other than malignancy. Mononucleosis needs to be ruled out by repeating the monotest. An otolaryngologist is best qualified to evaluate and advise you especially if the enlarged tonsils present a mechanical problem such as obstruction..

Other acute infections like strep can cause tonsil enlargement, but usually chronic irritation from either post nasal drip from sinusitis or allergy or from recurrent ge reflux/heartburn causes enlargement
