Yes it can. There are many potential issues with lateral patellar tilt. With tilting of the patella there is narrowing of the lateral aspect of the patellofemoral joint and subsequent patellar (and trochlear) chondrosis (AKA chondromalacia) which leads to arthritis. The tilt also predisposes to patellar instability and dislocation. You will also be prone to developing joint effusion and pain.
