My question is what causes swelling above the patella?

Several things....: 'swelling above the patella' needs to be seen by an ors. A true effusion or hemarthrosis will cause supra patellar swelling ( intraarticular).Meniscus tears, arthritis are common causes. Traumatic causes( injuries) include acl tears and patellar dislocations... Many other conditions( gout, for example) can cause large knee effusions. See a board certified ors. Best of luck!
