Cartilage loss: cartilage damage medially with some irregularities of the cartilage laterally

Chondrosis is cartilage disease and it is graded by radiologists reading MRI. The most widely accepted system is Outerbridge and grade 4 chondrosis is the highest grade = full thickness tear or cartilage loss with exposed bone. Otoh the heterogeneous lateral patellar cartilage is milder chondrosis and possibly outerbridge grade 1 or 2. Good luck, Dr. Brian Sabb
