Unclear cause: This sounds painful :( Presuming no actual nails are present, it depends on the location, how long you have had this, any prior injuries, any exacerbating maneuvers, and any other medical conditions someone may have. It needs a full medical evaluation, and sometimes x-rays. Depending on the suspected cause and location, an MRI may be a follow on test. I wish you well.
