Depends : You may need to ask them directly. Usually depends on your condition and various illnesses have certain routine checks. However if you've got an ongoing issue and you're not sure best to consult a doctor make sure the right diagnosis has been reached

Welk: You say you are chronically ill? What is your diagnosis. I would agree you would need monitoring but a lot would depend on your medical issues. So if you feel you need. Loser monitoring just call your doctor and speak to him sharing your concerns and let him explain your condition and your needs to care for you with the condition you have

Regular schedule: I try to see patients with chronic disease every 3-4 months. You did not mention which chronic diseases you are referring to; however, I would suggest at least 2-4 visits per year and discuss scheduling with your PCP.

VALID: Talk w yr GP : How often are yr routine appts? Some GPs sched ea mo for chronic; others 3-6 mo. More freq routine appts may help. May be hard for understaffed GPs w lots of sick pts. How often yr specialist visits? They usu sched 6-12 mo if yr stable. How easy to sched urgent, walk-ins? Expect GP to call you? Also, much extra stuff for a visit that's addt'l to the "face to face": GP wants to do more but can't.
