See a doctor : They are multiple factors that can affect the quality of sleep. Stress, diet, hormone imbalances, breathing problems may all contribute to to poor sleep. See a doctor who can take an adequate history, perform an examination and evaluate your concerns further. You may need referral to a sleep specialist as well

Difficulty falling: asleep is often associated w/anxiety while waking up early and not returning to sleep is associated w/depression. Periodic waking is associated w/overall light sleep. Please see you doc who may order a sleep study. A psychologist can help w/stress management. Peace and good health. Natural treatment includes melatonin supplement but w/professional guidance. Stop all stimulants after noon.

Seek help: Not sleeping well for 4 months is going to take a toll on you. Have you talked to your primary care doctor or have a sleep study done? If not get evaluated to find out the reasons. Have you practiced sleep hygiene -- going to bed at the same time; no naps in the afternoon, no caffeine after 2 or 3 pm; keeping your room on the chilly side, white noise machine? In any case, talk to your doctor
