Probably broken toe: You can bring your shoe to an old-fashioned shoe repair shop where they have "lasts" that they can fit into your shoes and stretch the leather. Those stores are getting hard to find because people are buying more sneakers and fewer good leather shoes. If your fifth toe is really angulated a lot, go to either to a podiatrist or a foot orthopedist.
