Good question: This is an ethical question. The appendiceal cancer could be carcinoid tumor, which is rare but usually not life-threatening. Parkinson's pts tend to worsen transiently post-op, getting very confused, etc. Sounds like a question for his oncologist. Quality of life is important. Thanks.

Advanced directives: I am sorry to hear about your grandfather. If he is able to participate in the decisions to be made regarding his care, then should assist in the process. If he is unable to do so, his health care surrogate should carry out your grandfather's wishes. Many of my patients in similar cases ask for comfort measures only or hospice care. In case of intestinal obstruction surgery may help. Best wishes.
