Delirium: Delirium is an altered mental state that is sometimes seen in patients in the hospital / ICU setting (ICU psychosis). Conditions that can mimic delirium can be emboli or stroke.

Infection, anesthesi: Elderly people often have a reaction to anesthesia similar to psychotic symptoms or confusion. Need to wait until it clears. Elderly people often exhibit psychotic symptoms or confusion when they have an infection. So given the recent surgery, would assess for infection and observe symptoms.
