Ankle arthritis: After any type of fracture or severe injuries there is the possibility of arthritis setting in. When you here or feel grinding this is a high likelihood that you have arthritis. I would recommend getting x-rays and possibly and MRI to see the extent of the damage and following up with a podiatrist trained in ankle surgery or an orthopedic surgeon to discuss some teatment options including surgery.

Xrays: Need x-rays taken. You may have bone growth over ankle joint or what's called an otc. See foot& ankle specialist.

Maybe.: It's impossible to tell from a brief description like that. Xrays and possible more advanced (ct or mri) imaging may be needed, along with a physical exam. The good news is that not everything needs surgery...In some cases bracing, therapy, etc. Can help.
