Cancer or calcificat: It may represent a prostate carcinoma or a prostate calcification from a prior infection. You may want to consider a urology consultation.

Needs biopsy: Please make sure this mass gets biopsied. I assume a urolologist did a transrectal prostate ultrasound to find this mass. About 10% of prostate cancers are hyperechoic. Best wishes.
