Husband has high liver ammonia levels, they stay normal when hes on lactulose but he hasn't taken it for a while. does he have hepatic encephalopathy?

Need more info: Hepatic encephalopathy is diagnosed by mental status. If he is confused, disoriented, or abnormally sleepy, probably he has it. High ammonia can help confirm encephalopathy, but by itself doesn't make the diagnosis. However, that plus not taking lactulose probably mean he is at high risk for it. Check with his doctor ASAP.
