It's all in the ches: All the conditions that you mention - idiopathic pulmonary fibrosis, cardiomegaly (enlarged heart), and gastritis / acid reflux affect the chest. The simplest answer is usually correct. I suspect that IPF has caused pulmonary hypertension (high pressures in the lungs) which causes an enlarged heart.
