See psychiatrist : You likely need to find the right combo of medication to begin with and then once stable you could include therapy. Try finding a Psychiatrist to work with.

Psychotic depression: Consider going a member of the to the psychiatry department of the nearest medical school. Also be evaluated for vitamin deficiency as Vit D, B6 can all have effects on mood and be evaluated for sleep apnea which can affect cognition and mood. Doctors prescribe medications but to discuss your issues you may also benefit from the input of a licensed social worker or psychologist.

Alternate treatment: Psychiatrist usually prescribe a combination of an antidepressant with an anti-psychotic medication. If that is not helpful the most effective treatment is Electroshock treatment ( ECT ) or Trans Magnetic Stimulation (TMS ).
