NEUROPATHY: People with diabetic neuropathy evolving into a Charcot ankle may not feel pain. For people with no health issues, I would think pain would be a presenting symptom.

Possible: Hello, It is possible if there are other comorbidities associated with your health. Otherwise pain would be accompanied with such a fracture.

Unless: you have a neuropathy(e.g. due to diabetes) , are anesthetized or under the influence of analgesic drugs, or have some other pathologic process in which you are desensitized to pain or unable to feel pain, you will feel pain if you fracture your ankle.
