Recurrent sinus, respiratory, uti infections.igm high at 392 mg/dl & ige high at 222 ku/l. no pneumococcal titers without vaccine. any idea of cause?

Immunology consult: This is a very complicated area. Not to side-step your question, but you need to consult with an immunologist. There are many more avenues to pursue, labs to analyze and physical exams to do to properly diagnose you. The acute infections will cause igm to be high, and high ige suggests an allergy component, but there can be more variations.
