Need to be seen: Rather than ask you to characterize your heart pain and skipped beats -- since you have a pericardial effusion and a window made to relieve it, you'd do well to go straight to a physician. The pain may be nothing but late-healing of nerves, and the skipped beats from coffee, but since you've been a pericarditis patient, you need a physician to see you. Good luck.

See a cardiologist: You may have recurrent pericarditis (inflammation around the heart). If your hole is large enough it may be putting strain on the heart causing these symptoms. It may also be simply something unrelated and benign. However with your history, i suggest you see a cardiologist.
