Rcfe resident under hospice supervision for over a year. needs to be in a snf but conservator refuses. reasons?

I am concerned : Please have Hospice social worker talk to conservator and get to understand the conservator's point of view and if the medical team feels that patient's care is compromised in RCFE, try if there are other support be offered through Hospice to keep him/her in the same surrounding.
