May still be gout: This could still be gout or another crystal induced arthritis. In order to tell you will need to see a doctor who can take fluid out of the toe when it is swollen and check it under a microscope. There are also many other types of arthritis and some non arthritis diseases which can affect someone your age and cause toe swelling. If your family doctor is unsure see a rheumatologist next.

X-RAY: It could be degenerative joint disease; DJD; wear-and-tear arthritis. You should get an x-ray.
