Family history: Usually a family history (first degree relatives) of heart attacks or strokes can be a red flag for developing similar conditions in the future. This also applies for conditions such as diabetes, hypertension etc... The rule of thumb is if a serious condition runs in your family (1st degree relatives usually), be sure to mention it to your primary care doc, who would decide whether to screen you.
