My friend gets frequent pain in his left arm for two three years now. he fears he has a heart problem. what test will let us know the exact problem?

Not his heart: It's unlikely to be his heart because it's unlikelly to be going on for so long without either getting better or having a heart attack. There's no single test .He needs to be examined. That may yield the diagnosis. Otherwise, x-rays of the shoulder and spine may be helpful. (sometimes ct or MRI is needed).
