Pulse ox: In Respiratory failure you can have increased rr or decreased rr depending on the cause of respiratory failure. It basically means that your are not making enough oxygen to maintain oxygen in the blood. Your lungs could react by increasing the rr first to compensate but then they tire out and the rr goes down (worse situation).
