Loculated pleural: This is most likely related to infection unless a trauma has recently occurred and then this can be related to secondary infection of a pool of blood that collected in the chest called a hemothorax. If it is only 4 mm this is too small for aspiration and usually will be treated with antibiotics/observation and CT to rule out malignancy which I am assuming has been done.
