Could be: gout, pseudogout, infection, rheumatoid arthritis, nerve problem, circulatory problem, etc. As you can see there are many possibilities. Have your doctor take a look at it..

Varies: There are multiple reasons that a person might have pain in the great toe while wearing shoes that is relieved by shoe removal. These can include a neuroma, callus, corn, bone or joint deformity such as a hallux valgus deformity (bunion). An exam will likely show the cause and appropriate treatment options. Consider being seen.

Big Toe Joint Pain: I am assuming that it is pressure from the shoe. Try to skip the front eyelet over the big toe area. Also try to get some padding material that you can tape to your foot just behind the joint (towards the arch), not over the sore spot, to off weight the area. Hope this helps. Dr Rich Blake

Possible arthritis: Bunions and arthritis can be painful and could be the cause of your pain without an examination and possible imaging studies its difficult to tell exactly what's going on. Since the pain has continued for several months, I would recommend evaluation by a professional.
