Malabsorption: Bariatric surgery works by making us feel full (reducing the size of the stomach) and make some calories pass through. While a gastric sleeve's primary mechanism is through reducing the size of the stomach, malabsorption can occur. Consult with your doctor or a nutritionist to see if this is the culprit, and to be certain there is not another process at work.
