I am on nexium (esomeprazole) 40mg i have to take zantac 300mg with it if not i have extreme heartburn. sometimes swallowing problem if zantac isn't taken what.

Acid reflux: It appears that your acid reflux is not under good enough control, particularly if you are having some trouble swallowing. You should return to your gastroenterologist to consider altering your medical therapy. If the trouble swallowing persists, an EGD may be helpful for evaluation and potentially for stretching the esophagus. Continue following anti-reflux precautions. Good luck.
