Mri in '14 showed supraspinatus tendinitis. arthrogram now shows same and small labral tear. no overhead activities due to inability to raise arm. had subacromial decompression in '09. what may be causing ongoing tendinitis? surgery for labrum tear?

Possible cuff tear: You would need to see orthopedic surgeon for an examination. There are certain exam tests which are + when a labral tear is causing symptoms. You may have also had progression of your supraspinatus or rotator cuff tendinitis to a partial or full-thickness rotator cuff tear. Nonoperative treatment can include anti-inflammatories and subacromial cortisone injections. Physical therapy is helpful.

Orthopedic decision: If you do not feel you are getting all the answers you need you might take all your studies and get a 2nd opinion. Causes of tendinitis can be because of underlying autoimmune disease like Lupus, use of some ased on exam and medications like Cipro (ciprofloxacin) and steroids, and ongoing irritation like overuse of the particular tendon. Decision on surgery should be on exam + studies. PT helpful. See your ortho.
