Widely Varies: The fact is it may never follow that pattern. The early process of appendicitis involves swelling and inflammation which produces vague pain around the belly button. The classic migration of pain to the right lower abdomen is from irritation of the abdominal wall by the appendix. not every appendix touches the wall. some are tucked behind the colon. A retrocecal appendix.
