Umbilical Hernia: It is unlikely to be a cyst or appendicitis. appendicitis not only causes pain but fever, loss of appetite, nausea, etc.... it also will worsen in a short time. cysts tend to be short lived and resolve on their own unless very large. I would be concerned for an umbilical hernia which will cause pain that can radiate. worse with straining and better with rest. They are easy to find on exam.
