Look at the Child!: Those Vital signs are not good and without seeing the child I would suggest going to your nearest ER that also does Pediatrics if you do not have a dedicated Pediatric ER in your area. Hope all goes well. This is nothing to take short-cuts with.

SICK TODDLER: Although the heart rate is within the normal range for a child this age, I am concerned about the respiratory rate doing in the 40s; even in spite of fever. That abnormal vital sign a loan would warrant an emergency department visit. Any urgent care with qualified personnel would send this child to the emergency department anyway once the vital signs were known.
