During night: pain under left ribcage, radiates to left shoulderblade and back. bloodtest shows nothing about spleen or pancreas, what could it be?

Varies: Multiple potential causes, first priority would be to rule out a cardiac or pulmonary cause, which could potentially be life threatening. If that and GI ruled out, consider a spinal cause. An MRI can be helpful for that.
