Back pain relief: You seem to have a chronic back pain syndrome. You should have an EMG performed to rule out a definite nerve compression. You can also try nerve blocks normally performed by pain management or neurostimulator implants. If pain is significant and or causing weakness then surgical intervention may need to be considered. Other neuropathic meds are available.
