Ask your doctor: whether you should have responded by now. If you really have external otitis/"water on ear", then you would be applying the medicine to the outside. If there is too much swelling the medicine might not be getting down into the canal and a wick may be needed. If internal otitis media/"fluid in ear", 2-3 days max. If your whole ear is swollen, you may need a different medicine.
