See your doctor: Since the diverticulum has recurred and is now problematic, surgery is indicated. Any hardness of the mass is rare; such a finding should prompt immediate MD consult because it may be a cancer. Marsupialization is successful for small lesions towards the outside but most of them further up the urethra require some form of excision like partial ablation or complete excision. Good luck.

Needs exam: Some urethral diverticulums can be treated temporarily with antibiotics. Once it's no longer infected then surgical options can be considered to repair it
