Check with ENT: First contact the surgeon who did the surgeon. If he can't help you, then your best bet would be to contact an allergist who can evaluate the cause for the recurrence (assuming no further obstruction to sinus drainage from the surgery) such as allergic disease and impaired immune responses to account for the recurrence.
