Sciatica: It would be a little unusual in an 18 year old, but one possible cause is a "sciatica". The most common nerve root involved is L5 which follows the distribution you are describing. There are other possibilities as well, such as a tendonitis of the anterior tibial tendon; I would not hesitate to be seen if it persists.
