Meniscal injury: I have the same injury so I can attest as a patient. Injury to the medial or lateral meniscus, frequently caused by a twisting or cutting motion, can cause sharp pain on knee flexion. Partial or complete tear to the posterior cruciate ligament can also cause some knee pain, though it is usually accompanied by some swelling. If the pain is mostly localized to the anterior kneecap or just below
