May be Phlegm: Sometimes the sensation of bad breath comes from the throat. I have many patients who suffer from sinus drainage. This can have an odor and be noticed when he sneezes.

DDS first: Rule out the obvious first. When was the last time he had a complete clinical and radiographic examination completed? Once decay and periodontal disease is ruled out, there may be systemic problems that need to be addressed by your MD, such as sinusitis or gastrointestinal issues.

See Family Physician: Although the bad breath when sneezing may simply reflect gingivitis, inefficient digestion or tonsil stones, you should see your family physician to rule out any other serious condition like diabetes, etc.
