Wrong place to ask: You're wandering (and wondering) blindly. Your insurance carrier has policies for everything, probably including your situation. Check your insurer's website. If it's not there, call them and ask to be sent the relevant medical policy. It's likely that your doctor hasn't read it either. Your next move will be determined by the policy. Pay particular attention to "medical necessity" clauses.
