Tooth pain: When was the root canal done, and on which tooth? This requires a lot more information, including a clinical exam, pre- and post-op x-rays of the tooth that had a root canal, plus an exam and x-rays of surrounding teeth and jaw. You might want to see another endodontist, dentist or oral surgeon for a second opinion.

3D images: 3D images of good quality generally show root fractures although there are sometimes factors that prevent 100% accurate visualization of all slices of the root. There are also other reasons that you might be experiencing pain either on that tooth or an adjacent tooth. This requires a one on one evaluation. If your own dentists have no explanation and this continues, I suggest getting a 2nd opinion
