Mittelschmerz: While for most mittelschmerz (ovulation pain) alternates from side to side. Most women have a dominate ovary that they ovulate from much more often. PCOs patients don't ovulate often and endometriosis is more assoc with painful sex, pain just before and early in the period more then mid cycle
